Helping Youth Thrive After Residential Care
On the Way Home is a family-centered aftercare program that helps youth successfully transition from residential care back into their homes, schools and communities.
Developed by Boys Town in partnership with the University of Nebraska-Lincoln, this research-backed, evidence-based service supports both youth and caregivers every step of the way -- beginning before the first home visit and continuing for up to 12 months post-discharge.
Our goal is simple: to ensure lasting success at home, in school and in life.
How It Works
Early Support
Services begin while the youth are still in residential placement, helping
families apply new skills and prepare for a smooth transition home.
Ongoing Guidance
Our team stays involved for 6 to 12 months after discharge, providing
consistent coaching and community-based support to maintain progress and
stability.
Core Program Components
Parenting Support
Parents receive in-home
coaching based on Common Sense Parenting® techniques -- mirroring
what youth learned in care. Families also gain 24/7 access to guidance
for managing behaviors and creating a nurturing, stable environment.
School Support
Consultants partner
with teachers and school staff to monitor attendance, grades and behavior. They
advocate for youth, develop personalized interventions and strengthen
communication between home and school.
Homework Support
Families learn how
to set up a positive learning environment at home, complete with
homework routines, skill reinforcement and access to extra academic resources
when needed.
Who We Serve
In Iowa, On the Way Home is offered to youth and their caregivers referred by Juvenile Court Services in seven of the eight Judicial Districts.
