The Aftercare Program serves youth who are under the supervision of New York’s Administration for Children’s Services, Division of Youth and Family Justice, and who are in the process of returning from out-of-home placement to their homes, schools and communities in a positive way. It provides up to six months of In-Home Family Services both while the youth is still in residential care (a non-secure placement) and when they return home. The in-home services intensify as the youth nears reunification and then continue for three months after the youth is reunited with his or her family.
